Select PT Eval - In Clinic to view available openings.
Once you have selected your appointment time and provider, you will be prompted to submit your information to create your patient portal.
Add your credit card information to reserve your time slot.
Create your portal password by following the link in your confirmation email titled “Registration at Lady Bird PT.”
Complete your new patient paperwork ahead of your appointment, located in the Documents tab of your patient portal.
Online scheduling is not available for in-home visits. Please call the clinic to ensure you are in our service area and book your in-home visit.

Log into your patient portal. (Still need to set up your portal? Click here!)
Select “Book an appointment” on your Schedule tab.
Choose PT F/U Office as your service and select your provider to view their upcoming availability and book your appointment.

Lady Bird PT is out of network with all insurance providers. By booking your appointment, you agree to pay the full cost of the session ($225) at the time of your appointment. Itemized superbills will be available in your patient portal after payment is rendered to submit to insurance for reimbursement. Please check with your insurance to determine if you have out of network benefits.

The state of Texas allows PTs to evaluate and treat patients for up to ten business days without a referral, but we will need a referral on file to continue treatment at any appointments that fall outside of that ten day window.
If you haven't already done so, please reach out to your provider and ask that they fax us a referral to 737-207-9386. If you already have a referral, you can upload it to your patient portal in PtEverywhere under the Documents tab or email it to us at contact@ladybirdpt.com.
We can accept referrals from an MD, DO, NP, PA, CNM (certified nurse midwife), or DC (chiropractor).
We cannot accept referrals from other PTs, CPMs (certified professional midwives), nutritionists, personal trainers, doulas, etc.
